Phyllis Jean Hazen, age 67, of Carrollton, died Saturday, October 6, 2018.

Funeral services will be held at 10 a.m. on Wednesday, October 10, at Bittiker Funeral Home in Carrollton, where visitation will be held from 5 to 7 p.m. on Tuesday, October 9.

The family suggests memorial contributions be made to the HELP Services Cancer Fund or the First Baptist Church maintenance fund, in care of Bittiker Funeral Home at 1201 Highway 65 North, Carrollton, Mo., 64633.

Survivors include her husband, Jerry Hazen, of Carrollton; daughters, Angela Hermanson (Troy), of Cameron, and Nicole Miller (Eric), of Hale; one sister, two sisters-in-law and three brothers-in-law; as well as five grandchildren.
