Sedalia School District 200 kindergarten enrollment for the 2019-20 school year will be held 4:30 to 7:30 pm Tuesday, April 2, at Smith-Cotton High School.
Katy Trail Community Health will be there to provide complete health exams for students and immunizations for those who are not up-to-date on vaccinations.
Some minor screening will take place so you will need to have your child with you–
–Your child’s Birth Certificate Your child’s Social Security number (voluntary identification and program use.);
–Your child’s shot record; you may look up the required immunization records on our website at www.sedalia200.org, Click on “Department” and “Health Services”.
–Proof of residence , One of the following: Copy of Deed of Trust, or Copy of Rental/Lease Agreement, or Utility Bill, or Copy of Pettis County Paid (personal) Property Tax Receipt, or Letter from Homeowner if residing with another person and proof of residency of homeowner.
–Parents please bring a form of your own personal ID (driver’s license is fine).
State law does not allow any student who does not have all required immunizations up-to-date to attend school.
While at the enrollment we will give your child a vision screening and check their shot record.
If parents do not have proper documentation at the time of enrollment, they will be permitted to complete the enrollment forms and procedures; however the child will not be enrolled until all documents are on file.
